Immunization Requirements

New York State Public Health Law #2165 requires that ALL students born after January 1, 1957 and taking six (6) or more credits show proof of vaccination for measles, mumps and rubella (MMR).

You can obtain this information from the last high school or college/university you attended or your pediatrician or family physician.  Non-compliance with this law will cause you to be withdrawn from all of your classes. Tuition and fee payments will be forfeited, your records will be impounded and you will not be able to register for any more classes until this obligation is met.

Meningococcal Meningitis Requirements

New York State Public Health Law #2167 makes it mandatory for ALL college students taking six (6) or more credits on campus to receive information about the meningitis disease and vaccine.

ALL students MUST present proof of receiving the meningitis vaccine OR sign a waiver declining the vaccine.

At this time SUNY Sullivan does not require students to be vaccinated to attend classes; however, by law students must complete, sign and submit the following meningitis response form to health services.

View and/or print the Meningococcal Meningitis Fact Sheet .  Print the Meningitis Response Form and mail or fax it to health services.
